Documentation Needed for Irregularities
- Call Holly if you have any questions.
- Statement forms need to be printed, completed, signed, and then scanned.
- Complete the google form with as much information as possible. The information you provide is what will be used to complete the irregularity for the state. You will attach your scanned statements to the google form.
